Home Visits

Home visits are available to certain DVA card holders and plan-managed NDIS participants in Central Gold Coast area, and surrounds. No travel fees for locations within central Gold Coast regions (within 10km of practising location).

A travel fee of $1.20 per km outside of 10km range applies for areas outside of Central Gold Coast region.

Home visits are primarily offered for NDIS participants, DVA card holders or those who are unable to attend a clinic appointment and Telehealth is not suitable.

Rebates

Medicare

Medicare rebates are accepted as part of payment upon a GP referral.

Medicare rebates are available to clients who have a chronic condition and complex care needs being managed by a medical practitioner (GP) under a shared care plan or under both a GP Management Plan and Team Care Arrangement.
We offer face-to-face, video and phone consultations with a Medicare referral from your doctor.

Private Health

We do offer Private Health rebates to those who are eligible. Please check with your Private Health provider prior to booking an appointment to see whether you are eligible to be reimbursed for part of the consultation fee.

DVA

DVA Health Cards (Gold and White) are accepted as payment upon a GP referral.

NDIS

NDIS services available on the Gold Coast and/or Australia-wide via Telehealth.

Able to service plan-managed NDIS participants on request. Telehealth is available to service plan-managed NDIS participants Australia-wide. In-person NDIS service available on Gold Coast and surrounds, travel fees may apply.

NDIS Services offered

$194 – Initial consultation (60 minutes)

$280 – Initial consultation + report (60 minutes)

$140 – Review consultation (30-45 minutes)

$140 – End of plan report (includes participants progress, results and recommendations for future visits)

Travel fees

No travel fees for participants living in Gold Coast and surrounding areas. Travel fee of $1.20 per km outside of Central Gold Coast regions. Please contact below for more details.

How does our plan-managed NDIS services work?

Plan manager contact My Dietitian Clinic via enquiry form below requesting Dietitian services for an NDIS participant on the Gold Coast or Australia wide for Telehealth services
Dietitian responds and accepts agreement
Care plan arrangement sent to Dietitian and signed
Dietitian contacts the participant and arranges first meeting or telehealth appointment
Invoice for services is sent via email to plan manager

If you wish to cancel or reschedule your appointment, please let us know 24 hours prior to your appointment. You will not be charged a cancellation fee.
